Hospital Admissions
Great Lakes Family Care providers admit their patients to a hospital-based physician known as a Hospitalist. Hospitalists are specialists in Internal Medicine. Your primary care provider will contact, or be contacted by, a Hospitalist when you are admitted to the hospital. They will discuss your specific condition and medical history. The Hospitalist will visit you daily during your hospital stay, communicating with your primary care physician as needed.

Upon discharge, the Hospitalist will update your primary care provider on your care while in the hospital. Your primary care provider will receive a summary of all lab work, reports, and studies that were performed.
