Appointments
We understand that your schedule may change and you may need to cancel an appointment. Please inform us within 24 hours if possible by calling (231) 775-6521.

We require that services be paid in full on the day received, unless other arrangements are made ahead of time. Great Lakes Family Care works closely with several insurance carriers, so we can bill these carriers for you. However, any co-pay, deductible, or co-insurance amount must be paid on the date of service. Arrangements can be made for special circumstances. You will be expected to present your insurance card at each visit. Please direct any questions about your coverage or billing statement to (231) 775-6521.

If you are a new patient or have an appointment for an annual physical, we ask that you arrive 15 minutes before your scheduled time to fill out patient information forms. Also, please bring your medications so we may review them with you.

Great Lakes Family Care has an automated appointment reminder service that will call 48 hours before your appointment.

Great Lakes Financial Counselors are knowledgable of many insurance plans and are available to aid in understanding and using insurance benefits.
